10/17/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expression such as a smile on the face and engaging in general conversation with QMHPA. QMHPA inquired about her day. Kaila verbalized to QMHPA that her day was going well. QMHPA informed Kaila that this session was going to focus on improving her IDAL’s and ADL’s. Intervention: QMHPA began the session by inquiring about Kaila’s current thoughts feelings and emotions. QMHPA engaged Kaila in a discussion surrounding her medication management. QMHPA conducted a skills building activity with Kaila. QMHPA explained to Kaila that the purpose of this activity was to assist her budgeting techniques where she is to identify the lowest price item that will accommodate the desired budget. QMHPA explained to Kaila the importance of budgeting and understanding the proper use for money control. QMHPA engaged Kaila in a discussion surrounding future expenses and goals she would like to accomplish that involves money. QMHPA educated Kaila on how she will not be able become self-sufficient if she spends her money all at one time. QMHPA discussed proper ADL's through role play. QMHPA engaged Kaila in a discussion that surrounded her ADL's and what are the objectives of her ADL's. QMHPA conducted a skills building activity. QMHPA explained to Kaila that the purpose of this activity was to assist her with how to properly placing her clothes in the washing machine and using the correct amount of washing detergent. Response: Kaila appeared to be receptive to QMHPA’s interventions and services that were provided to her during the session. Kaila verbalized to QMHPA during the session that she has been adhering to her medication what little she has left. She discussed with QMHPA that she cannot wait until she goes to her appointment later on in the month so that she can have a full supply of her medication and not stretching them out because she doesn’t have a refill. When reviewing her ADL’s and IDAL’s Kaila needed help with those objectives for she noted to QMHPA that she was working on the betterment of increasing those objectives as well. As mentioned in a previous session, Kaila verbalized to QMHPA that she does struggle with being able to wash her clothes and how to wash them because she at times will fade her clothes. Kaila thanked QMHPA for assisting her with this skill and indicated to QMHPA that she is going to continue to need assistance with this skill.
